Eligibility Requirements:
- Care recipient is a Frederick County resident.
- Care recipient must be either…
- 60+ or have a dementia diagnosis
- Adult (age 19–59) with a disability who is being cared for by a relative caregiver over age 55 that is not the child’s natural or adoptive parent
- Care recipient requires assistance with at least 2 Activities of Daily Living.
What is Eligible for Reimbursement?
- Short-term respite provided at an adult day center, in-home, or in a licensed facility such as an assisted living facility or nursing home
- Eye exams, prescription eyeglasses
- Dental exam, dentures
- Hearing aids & hearing aid batteries
- Personal emergency response systems
- Transportation costs for medical appointments
This grant is designed to provide family caregivers temporary relief from their day-to-day caregiving responsibilities. Subsidies will be available until grant funds are depleted, first come, first serve. You can find the application on our website or contact us to receive a copy.
